Add missing package versions to web.csproj

The PackageReference items were missing version specifications:
- Microsoft.VisualStudio.Azure.Containers.Tools.Targets (added 1.21.0)
- Yarp.ReverseProxy (added 2.2.0)

This fixes the NuGet error NU1015 that prevented Docker builds from
successfully restoring package dependencies.
